Custom forms for nonprofitsEvery form fills your CRM, not a spreadsheet.
Build a form for anything, from volunteer sign-ups to surveys. Embed it on your site, share a link, or open it from a button. Every answer lands on the person’s profile in your CRM and can start your follow-up automatically.
Pro from $600/mo. Card required, cancel anytime before the trial ends.
Forms that work with the rest of CauseVox
Every submission lands in the same CRM as your donors and guests, so a form is the start of a relationship, not a file to import.
Right now, every form ends in copy and paste.
If your sign-ups come in through a separate form tool, you know where the afternoon goes.
Answers stuck in a spreadsheet
Each form has its own spreadsheet, and nobody on your team knows which one is current.
Retyping into the CRM
Someone exports the sheet and types it into your donor database, and the same person ends up in there twice.
Nobody followed up
The volunteer who raised their hand three weeks ago still hasn’t heard back, because the list sat in someone’s inbox.
Build it once. Let every answer file itself.
Build the form in CauseVox, put it wherever people will see it, and let each submission land in your CRM and start the follow-up.
Build the form you need
Add the questions you need, drag them into order, and set any two side by side. Add headings and short notes so longer forms stay easy to fill out.
Put it wherever people are
Paste one snippet on your website, share a direct link, or add a button that opens the form in a pop-up. Match your fonts and colors, or make the background transparent so the form blends into the page.
<div class="cv-embed-…"></div> secure.causevox.com/form_direct… Follow up automatically
Pick an email list and every person who submits joins it. Send a confirmation email, show a thank-you message or send them to your own page, and start any automation, like a welcome series for new volunteers.

Common questions
Can I put a form on my own website?
Yes. Add the CauseVox code to your site once (or ask whoever manages your website), then paste a short snippet wherever you want the form to appear. You can also share a direct link, or add a button that opens the form in a pop-up. Match your fonts and colors, or make the background transparent so the form blends into the page.
Will it work on my website?
It works on any website that lets you add a snippet of code. If you can’t edit your website, you don’t need to: host the form on a page you build in CauseVox, or share its direct link.
Where do the answers go?
Every submission is saved on the form and matched to a contact in your CRM by email. You can also save any answer into a field on the contact’s profile, so it shows up there too. If a new answer replaces one already on file, the change is logged for review.
Can a form add people to my email list?
Yes. Pick an email list for the form and everyone who submits it is added automatically. You can also start any automation when a form is submitted, like a welcome series for new volunteers.
Can I send a confirmation email?
Yes. Send every person a confirmation email the moment they submit. After they submit, you can also show a thank-you message on the form or send them to a page on your own website.
Do I have to ask for a name?
No. You can require first name, last name, and email, make them optional, or ask for just an email. The email is what links each submission to a contact.
Which plan includes custom forms?
Custom forms are part of Pro, from $600/mo billed annually. You can try Pro free for 14 days. Card required, cancel anytime before the trial ends. See pricing for every plan.
